Live Trained LLC and/or Live Train LLC (“Live Trained”) provides coaching services (“Coaching Sessions”) for various types of activities by connecting participants with qualified coaches to train the participants in that Coach’s area of expertise. Some of the activities Live Trained provides Coaching Services for involve significant risks of injury including the potential for permanent disability and death. For example, activities such as MMA, boxing, or wrestling pose a significant risk of injury to participants even when everyone is following required safety protocols and exercising due care in a reasonably prudent manner.  
In light of these risks, and in consideration of Live Trained allowing my child/ward to participate in Coaching Sessions, for I myself, my spouse, my child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, the undersigned acknowledges, appreciates, and agrees:1. There are significant risks of injury to my child/ward in connection with their participation in Coaching Sessions, and while particular rules, equipment, precautions, and personal discipline may reduce these risks, the risks of serious injury still exist. Despite these risks, I intend for my child/ward to participate in the Coaching Sessions.2. I have explained to my child/ward and he/she understands the risks associated with the Coaching Sessions, his/her responsibilities for adhering to Live Trained’s rules and regulations, and the appropriate safety precautions my child/ward should take to help prevent injury.3. To assume all such risks, both known and unknown, even if arising from the negligence of the Releasees (defined below), or others and assume full responsibility for my/our child/ward’s participation in the Coaching Sessions.
